Here's everything you need to know before your first appointment.
Types of Lip Enhancement: What's Actually Being Injected?
The vast majority of lip enhancements use hyaluronic acid (HA) filler — a gel that is naturally compatible with body tissue. HA fillers come in different formulations with varying thicknesses and consistencies, and your provider will choose the product that suits your lip anatomy and desired outcome. In the lips specifically, lighter formulations are often preferred to maintain softness and natural movement, while slightly denser products may be used to define the border.
You may have heard of different techniques: the Russian Lip technique, for example, which uses a series of vertical injections to create a flat, projected, heart-shaped result — popular on social media but not universally flattering and requiring considerable injector expertise. A more classical approach places filler to enhance the natural volume and definition of your lips while preserving their movement and proportion. It's also worth noting that lip enhancement isn't only about adding volume. Your Consultation: What We Discuss Before a Single Needle Is Picked Up
At The Refined Co, no lip treatment begins without a thorough consultation. This isn't a formality — it's where we do the most important work. We start by discussing your aesthetic goals: are you looking for subtle enhancement that just makes your lips look a little more defined? More hydration and structure? A noticeable volume change? And equally important: what don't you want? Showing reference photos is entirely welcome and genuinely helpful.
We also assess your lip anatomy in detail — the ratio of upper to lower lip, the definition of your Cupid's bow, any asymmetries, and how your lips relate to your other facial features. Beautiful lips aren't a fixed measurement; they're proportional to your face. Your nose, philtrum length, and facial thirds all influence what "balanced" looks like for you specifically. A comprehensive assessment allows us to recommend the right product, the right amount, and the right technique — rather than applying a standard protocol to every patient.
We'll also review your medical history, discuss any blood thinners or supplements you're taking (which can increase bruising risk), and answer every question you have before moving forward. There is no pressure to proceed on the day of your consultation if you'd like more time to think.
The Treatment: What Actually Happens
Topical numbing cream is applied to the lips and left on for 30 minutes before treatment begins. This is not optional — it's standard at The Refined Co because a comfortable patient is a still patient, and precision matters. Most HA lip fillers also contain lidocaine, which further reduces discomfort during injection.
The injections themselves take 15–20 minutes. You'll feel pressure and mild pinching — most patients are surprised by how manageable the sensation is once the numbing has taken effect. Immediately post-treatment, the lips will appear more swollen than the final result. We assess symmetry and distribution at the end of the session and make any immediate adjustments before you leave.
You'll be given an ice pack for the first hour to reduce swelling, and we'll walk you through aftercare instructions before you go. We want you to leave feeling informed, not just treated.
After Your Appointment: Swelling, Bruising, and the Two-Week Rule
The first 24–72 hours after lip filler are the most dramatic in appearance — and the least representative of your final result. Swelling is normal and expected. Your lips will look larger than they will once settled, and they may feel tender and firm. Bruising is possible, particularly if you had any blood thinners, alcohol, or supplements like fish oil and vitamin E in the days before treatment (which is why we advise avoiding them for 48–72 hours pre-appointment).
Most swelling resolves within 3–5 days. Residual subtle swelling can persist for up to two weeks, which is why the standard advice is to assess your final result at the two-week mark — not the day after. If there's any asymmetry or area you'd like adjusted, that two-week appointment is when we evaluate and address it.
Aftercare is simple but worth following: avoid strenuous exercise on the day of treatment (increased blood flow increases swelling and bruising risk), no extreme heat (saunas, hot yoga) for 24 hours, avoid sleeping face-down the first night, and keep SPF on any exposed area. Protect the result by keeping the lips hydrated.
How Long Does Lip Filler Last?
Lip filler longevity varies from person to person. The lips are a high-movement area, and HA filler metabolizes more quickly here than in other parts of the face. Most patients find their lip filler lasts 6–12 months. Factors that influence this include your individual metabolism, the specific product used, the amount injected, and how active your lifestyle is. Patients who exercise frequently or have faster metabolisms tend to metabolize filler more quickly.
With consistent maintenance appointments, many patients find their results last longer over time, as the tissue adapts and a smaller amount of filler is needed to maintain the look. First treatments often require slightly more product to achieve the desired outcome; top-up appointments are typically more efficient.
Addressing the "Filler Face" Fear
One of the most common concerns we hear from first-time lip patients is some version of: "I don't want to look done. I've seen those Instagram lips and that's not what I want." We hear this, and we agree — that aesthetic isn't what we practice or what we recommend. Overfilled, duck-shaped lips result from too much product, the wrong product, poor technique, or a combination of all three.
The most common mistake in lip enhancement isn't what's injected — it's how much. A conservative approach, starting with less than you think you want and building over multiple sessions, almost always produces better results than a maximalist first treatment.
